Jonathan Levin Scholarship Program

Established in 1997, the Jonathan Levin Scholarship Fund celebrates the all too brief life of a caring and dedicated teacher, committed to the welfare of children and to equal opportunities for both learning and recreation.

2023 - 2024 SEASON

SCHOLARSHIP Applications are being accepted. 

Please print and complete the form above. Hard copies of the application form are also available at Riley Rink at Northshire Civic Center.

Scholarship applications can be submitted anytime. Please complete the entire application.

Scholarships are awarded on first come, first served basis in accordance with program availability, participant eligibility and available funds. 

Requirements:

  • Online registration required for a specific program.
  • Pre-school up to 8th grade only.
  • Proof of Income: MUST include last year’s tax return.
  • Hardship Letter (encouraged but not required) – please attach separate letter.

Additional Information:

  • The maximum Scholarship per household is $600, NOT per child.
  • Scholarships may be requested for program fees and/or equipment.
  • Scholarship awards may be granted to families who include a hardship letter but do not qualify based on income.
  • Parent volunteering is highly encouraged.
  • Your information will be reviewed and kept strictly confidential.
  • You may be asked to provide additional information or documentation at any time during the review process, or at a later date.
  • Incomplete applications will delay processing and/or disqualify the application.
